CAT – Spay/Neuter Request Form

As of right now this program has been put on hold. During the Vet Voucher Program, the Cheboygan County Humane Society will donate $90 towards the spaying (female) and/or $60 towards the neutering (male) of your cat(s). If you are a resident of Cheboygan County please fill out the following form to request a voucher. Note you will be responsible for any additional vet charges on your bill. We will not honor voucher requests for same day appointments.
